Health District Community Health Centre closer to the bereaved
or, in exceptional circumstances, at their home.

Eligibility Criteria
• Family and friends of anyone who was registered with
the SWS Local Health District Palliative Care Service
and died at home, in a hospital or a nursing home.
• Bereaved family and friends of anyone who died in another Palliative Care Service
but lives within SWS Local Health District.
